Role:
The Admissions Coordinator communicates and assesses patients in their place of residency to ensure a smooth transition of care to our Care Team. This person assumes the responsibility of the coordination of an effective and efficient verification process 


Job Responsibilities:
  • Handles and communicates initial inquiries for care with prospective patients
  • Coordinates verification process for all referrals
  • Facilitates patient referrals overall
  • Completes assessments and admissions of care in prospective patients' place of residency:
    • explains services 
    • addresses and educates on any and all questions
    • completes necessary documentation to meet legal and regulatory requirements
    • completes documentation according to company policy
    • develops plan of care for non-clinical or non-skilled patient assessments/admission
    • coordinates needs to nursing team with any and all skilled assessments/admissions 
  • Acts as company representative in all workplace and remote settings
  • Communicates effectively with nursing team to develop full patient assessment
  • Develops and fosters relationships with potential referral sources
  • Other duties as assigned by leadership team

Requirements:
  • 1 year experience in Home Health
  • Valid driver's license and auto insurance
  • Reliable transportation
  • 1 year experience in sales
